How to Send Your Out of Network Dental Claim (An Easy Step-by-Step Guide)
If our office is "out-of-network" with your dental insurance, it just means we do not talk directly to your insurance company. Instead, you will be the messenger. Not every insurance company will pay for out-of-network services, but many will.
You will pay us directly for your visit, and then you will ask your insurance company to pay you back.
It might sound scary, but don't worry. It is just like filling in a simple puzzle. We give you all the puzzle pieces before you leave our office.
Here is how to do it in 3 easy steps.
Step 1: Gather Your Tools
Before you start, make sure you have these three things at your table:
Your Insurance Claim Form: You can print this from your insurance company’s website.
Your Detailed Receipt: This is the detailed receipt we gave you at the end of your visit.
A Pen: Blue or black ink works best.
Step 2: Fill Out the Form (Line by Line)
Most dental forms look very similar. They are broken up into numbered boxes. Grab your detailed receipt from us, and let's copy the answers over.
Boxes 1 to 11: All About The Policyholder. This part is about the person who owns the insurance. If you get insurance through your job, this is you. If you get it through a spouse of family member, then this part is about them as the policy holder. Write the policy holder full name. Write their home address. Write their birthday. Write down their Member ID number. You can find this on your insurance card.
Boxes 12 to 17: All About the Patient
This part is about the person who sat in the dentist's chair for service. If you were the patient and the policy holder, just check the box that says "Self" and write your name again. If the patient was your dependent or on your policy, write their name and birthday here.
Boxes 24 to 35: What Procedures were done
This looks like a big math grid, but it is super easy. You are just going to copy exactly what is on the receipt we gave you.
Date of Service: When did you visit us?
Tooth Number: Did we fix a specific tooth? Copy the number from our receipt.
Procedure Code: This is a 4-digit code that starts with the letter D. This code tells the insurance what we did (like a cleaning or a filling). Just copy the code from our receipt.
Fee: How much did it cost? Write the price from our receipt.
Boxes 36 and 37: Your Signature (Important)This is where you promise that everything is true. Sign your name and write today's date.
***Super Important: Look for a box that says "Authorize Payment to Patient/Subscriber." Make sure you sign this. This tells the insurance company to mail the money straight to YOUR house, not to us.
Boxes 48 to 58: All About Our Dental Office
This is where you tell the insurance company who did the work. Once again, just copy this directly from the receipt we gave you. Write the dentist's name.
Write our office address and phone number: 1526 Walden Ave, Cheektowaga NY 14225. 716-882-3777.
Write your Provider's Number (called an NPI number).

Step 3: Mail It In. You did it! The hard part is over. Now, just fold up your Insurance Claim Form plus the Detailed Receipt we gave you. Put them both into an envelope. Put a stamp on the top right corner. Write the insurance company's address on the middle of the envelope (you can usually find their mailing address on the back of your insurance card). Drop it in your mailbox and you are done.
